To report a hazard at JBLE, you can call, or email the Safety Office. If contacting by email, please use the AF Form 457 or the DA Form 4755. 

Upon receipt, the Safety Office will conduct an investigation within 24 hours for life-threatening hazards. Other hazard investigations can take up to 10 working days. Upon conclusion of the investigation, the submitter will be notified of the results.
 

Definition of a Mishap: an unplanned occurrence, or series of occurrences that result in damage or injury.

Damage or Injury includes:

1. Damage to DoD property (not normal wear and tear)
2. Injury to DoD military personnel on/off-duty
3. Occupational Illness to DoD military/civilian personnel
4. Injury to on-duty DoD civilian personnel
5. Damage to public/private property,  or injury/illness to non-DoD personnel caused by USAF operations

Examples of what to report:

1. Loss of consciousness
2. Physician ordered days off from work
3. Restricted work or transferred position
4. Greater than first aid treatment
5. Vehicle related incident
6. Damage to government property

NOTE: Civilian mishaps that occur OFF-DUTY do not need to be reported.

Complete the Supervisors Mishap Report (AF Form 978) then send via email to the 633 ABW Safety Office.

Risk Management, at work and at home, is the foundation of the JBLE Safety Program. This decision making process is a way to look at and evaluate challenges.

Four Principles of Risk Management:

1. 1. Accept no unnecessary risk
2. 2. Make risk decisions at the appropriate level
3. 3. Integrate risk management into operations, activities and planning at all levels
4. 4. Apply the process cyclically and continuously

Smoke Detectors save lives! Early warning gives you and your family time to escape.  Practice your fire escape plan with your family, help your children learn to Get Out, Stay Out!

Remember:

• • Test smoke detectors monthly.
• • Replace the battery at least once a year. If the alarm makes a "chirping" sound, replace it immediately. 
• • Replace all smoke alarms every 10 years.
• • Smoke alarms should be located in every bedroom, in common areas on each floor of a home. Mount them at least 10 feet from the stove and less than 12 inches from the ceiling, away from windows, doors and ducts.
• • Be sure to purchase smoke alarms with the label of a reputable testing agency, like Underwriters Laboratories.

Traffic Circles are as easy as 1 - 2 - 3!

1. 1. Slow Down - By entering the circle at a slower speed, crashes that do happen are much less severe.
2. 2. Look Around - Drivers should choose the lane for where they want to go after the circle. Signs and markings help you select a lane.
3. 3. Be Ready to Yield - Drivers must yield to traffic in all lanes of the traffic circle, not just the lane closest to you.
    

Are you interested in becoming a motorcycle rider? Talk with your unit's Motorcycle Safety Representative or Motorcycle Mentor on how to become a DoD Motorcycle Rider.

The following Personal Protective Gear is required when riding:

• DOT approved helmet.
• Goggles, wrap around glasses, or a full face shield that meets ANSI Z87.1
• Long sleeve shirt/jacket.
• Sturdy, over the ankle footwear.
• Well-fitting, full-fingered gloves.
• High visibility upper garment (recommended)

Steps to becoming a rider. 

1. Airmen meet with your MSR and register in AFSAS/MUSTT / Soldiers meet with your units Motorcycle Mentors.
2. All military members must take an approved basic riders course.
3. Upon completion of a BRC, register to take the BRC-2u or ARC course hosted by the 633 ABW Safety Office.
4. Document your annual motorcycle safety briefing.

All military riders must take an approved motorcycle safety course every five years.

Tips for Hunters:
Review and follow the four basic rules of firearm safety. The most common hunting incidents result from hunter judgment mistakes. To avoid hunting incidents, every hunter should review and follow these basic rules:

1. 1. Treat every gun as if it is loaded
2. 2. Never let the muzzle of a firearm point at anything you do not intend to shoot
3. 3. Keep your finger off the trigger and out of the trigger guard until your sights are on the target and you are ready to shoot
4. 4. Always be sure of your target and what is beyond 

Wear blaze orange clothing.  According to the National Rifle Association, deer cannot distinguish this color, but your fellow hunters can, and for that reason, wearing blaze orange helps enhance safe hunting.  43 out of 50 states require wearing blaze orange during specified hunting seasons.

Remember common sense safety rules. When you go hunting, keep other common sense safety rules in mind:

1. 1. Check the weather report before you go
2. 2. Tell someone, in writing, where you will be and when you will return
3. 3. Carry a first aid kit
4. 4. Bring dry clothing
